• No denial of services that were prior authorized
• 48-hour turnaround for peer-to-peer reviews; 24-48 turnarounds following adverse determinations and appeals.
• Clear rules for unanticipated services and adverse determinations
• Transparency in policy changes and consistent PA timelines
• PA stability and portability across plans
