Health Promotion of complementary and Alternative Medicine (CAM) in Physical Therapy Survey

Dear Prospective Participant,

Students from the Indiana State University Doctor of Physical Therapy program are conducting an anonymous survey titled, “Health Promotion of Complementary and Alternative Medicine (CAM) in Physical Therapy”.  The purpose is to identify if physical therapists personally practice CAM as well as measure the physical therapists’ knowledge, beliefs, self-efficacy, and utilization of CAM methods in their practice. To participate, you must be 18 years or older, a member of INAPTA, and a licensed and practicing physical therapist. The survey is voluntary.

Since your answers are to remain anonymous, PLEASE DO NOT PUT YOUR NAME ON THIS SURVEY. The survey will take 5 minutes.  Please answer the questions to your comfort level. The results will be reported for the group of respondents as a whole.
